What vascular pathology is related to Diabetes?
Small vessel: hyaline arteriolosclerosis
Large vessel: Artherosclerosis
What is the most common cause of death in diabetes?
MI
What is the pathophysiology of neuropathy and cataracts in diabetes?
Sorbitol accumulation due to aldose reductase and decreased or absent sorbitol dehydrogenase
What are lab tests used to confirm diabetes?
Fasting glucose (>120), glucose tolerance test (> 140 at 2 hrs, > 126 overnight), and HbA1c (>8 is a predictor of last 3 months)
What is the pathology of DM 1?
Autoimmune against B-cells
What is the pathology of DM2?
Insulin resistance, progressive pancreatic failure
Does DM1 or DM2 have a higher genetic component?
DM2
What are the HLA associations with DM1?
HLA-DR3 and 4
Is ketoacidosis more common in DM1 or DM2?
DM1

What is the pathophysiology of diabetic ketoacidosis?
Increased stress (infection) –> increased fat breakdown –> increased ketogenesis –> Increased ff. –> ketone bodies produced
In what form are the majority of ketoacids produced in DKA?
B-hydroxybutyrate > acetoacetate
15 year old presents with Kussmaul respirations, nausea,/vomting, abdominal pain, psychosis, and fruity breath smell?
DM1, DKA
What is the treatment for DKA?
IV fluids, IV insulin, K+, and glucose (if necessary)
