What life style advice should be given for constipation?
increase fibre, exercise, increase fluids
How to treat acute & chronic constipation?
bulk forming laxative
add or switch to osmotic laxatives
add stimulant laxative
prucalopride for women
How to treat opioid induced constipation?
avoid bulk laxatives and use osmotic & stimulant ones
oxycodone with naloxone

When should a patient be immediately referred relating to GIT
persistent unexplained change in bowel habit
palpable mass in lower right abdomen or pelvis
rectal bleeding
narrowing of stool calibre
fam history of colon cancer or IBS
unexplained weight loss, iron def anaemia, fever or nocturnal symptoms
severe constant constipation which can’t be treated
What is haemorrhoids?

increased intra-abdominal pressure

how to treat Haemorrhoids?
Laxatives if constipated
analgesia for pain/ topical hemorrhoidal prep
What are red flag symptoms of diarrhoea?
blood in stools recent hospital treatment/ antibiotic treatment constant vom weight loss painless high water diarrhoea nocturnal symtoms
How would you manage acute diarrhoea?
avoid contact with people for 48 hours

What is IBS? and how is it characterised?
chronic relapsing and often lifelong disorder of GI function
abdominal pain

what are the red flags for IBS
unexplained weight loss rectal bleeding frequent loose stools for more than 6 weeks over age 60 abdominal mass rectal mass anaemia fam history of bowel or ovarian cancer
treatment for IBS?
antispasmodics - mebeverine, alverine, peppmint oil
dietary & lifestyle advice
constipation - lax
diarrhoea - antimotility
then low dose tricyclic antidepressant - amitriptyline 5-10mg at night (pain relief)
selective serotonin reuptake inhibitor - citalopram
