Optimise blood sugar levels and blood pressure
ACEi are the treatment of choice in diabetics for blood pressure control (should be started in all patients with diabetic nephropathy even if normal BP)
Control dyslipidaemia

Screen annually using urinary albumin:creatinine ration
Should be an early morning specimen
ACR>2.5 = microalbuminuria
