What is the process for administering a CD?
(P)Check prescription
(S)Check stock level in locked CD cupboard
(E)Enter date, time, patients name, amount given, form and remaining balence in the CD record book.
(P)Prepare drug and take to patient with CD record book and drug administration chart
(A)Check and administer
(S)Sign CD record book and drug administraion chart
Two signatures required and any excess/waste must be recorded appropriately (specific CD disposal protocol and method)
Why is it important that the nurse in charge of the ward has possession of the keys?
They are responsible for them until the next shift.
Requisitions must contain
Hospital Ward Drug Name, Form, strength, ampoule size if more than one available quantity signature and printed name of nurse date signature to receive goods for transit signature for receipt at ward
As soon as possible after delivery, the registrant in charge should
check the CDs against the requisition
if this is correct, sign the pink ‘received’ sheet
place the CDs in the CD cupboard
note the CDs into the record book
check that the new balance is consistent with actual stock levels
What should happen when CDs arrive at the ward
Handed to a designated person (see local policy)
General guidance for the storage of CD drugs
Cupboards must be locked when not in use.
The lock must be unique
keys must only be available to authorised members of staff
No other medicines can be stored in the same area.

date name amount form name/signature of issuer name/signature of witness balance of stock
Details recorded when CDS are returned to the pharmacy (carbon book?) + CD record book
date name, form, strength and quantity of drug being returned reason for return name and signature of pharmacist name and signature of nurse witness
