A
- Name and registration #
- The practice address
- Name of patient
- Date of issue
- Date of expiry
- Optom signature
- Brand name of the lenses
- Base curve
- Total diameter
- Back vertex power
- Must state it is a contact lens rx.

Which patients are classified as high risk?
A
- Poor compliance
- Extended wear
- Continuous wear
- Diabetes (they have higher risk of infection)
- Smokers
- History which may put them at higher risk
- Keratoconus
- Ortho-k
- Scleral contact lenses
- Dry eye symptoms
You may extend trial period out to two lens life cycles
Could be up to 6 months for others

What is the process/routine at an aftercare appointment?
A
- Patient interview
- Assessment of vision
- Overrefraction and binocular balance
- Assess lens fit
- Observe Px remove lens
- Slit lamp assessment of anterior segment and tear film
- Summary of outcomes and re-educaiton of lens care and wear

Purpose of aftercare appointment?
A
- Identify difficulties with eyes and lenses
- Assess satisfaction with lenses
- Check patient compliance and lens use and care
- Review lens fit and determine if modification required
- Monitor ocular condition, review for phsiological changes
- Re-educate and minimise risk
