Explain where to find the qualification requirements for the contractor’s SSHO.
Qualifications are located in the UFGS 01 35 26 under the paragraph titled Personnel Qualifications.
List the (10) required sections of the contractor’s APP.
Signature sheet, background information, statement of safety and health policy, lines of authority, list of
subcontractors and suppliers, training, safety and health inspections, mishap reporting and investigation, individual health and safety plans, and AHAs.
Explain what an AHA is.
The AHA defines the work sequence, anticipated hazards, conditions, equipment, materials, personnel and the control measures implemented to eliminate or reduce each hazard to an acceptable level of risk. The contractor performing each DFOW must prepare
and submit the AHA to the government a minimum of 15 working days prior to beginning the work.
Describe what CIRS is and what It’s used for.
CIRS is an online computer application used to
track safety incidents and near misses. The results of accident investigations performed by the contractor on all recordable injuries, illnesses, property damage or near misses as defined by EM‐385‐1‐1 must be entered into CIRS.
List the (5) high risk work activities that require a competent person and where to find the qualifications for the competent person.
Confined space, crane and rigging, excavation/trenching, fall protection and scaffolding. Competent person qualifications are listed in UFGS 01 35 26 and in EM385‐1‐1.
List at least (5) safety related documents the contractor must maintain at the jobsite per UFGS
01 35 25 or EM‐385‐1‐1.
The contractor must maintain an accident prevention plan (APP), activity hazard analysis (AHA), safety deficiency tracking log, subcontractor DART/EMR rates, latest completed safety self‐evaluation checklist, documented daily equipment inspections and OSHA
300 log.
Describe the process for the contractor to obtain a hot work permit and when it is required.
Hot work permits are issued by the base Fire Marshall. Contractor must submit request directly to the Fire Marshall whenever work involves burning, grinding, welding, torching or cutting.
