HRA
Health Risk Assessment
A thorough assessment found in the EMR comprises family history, active medical conditions, social history, review of symptoms, fall risk and self-assessments.
CMS
Centers for Medicare and Medicaid Services
Federal agency in the US department of Health and Human Services that administers Medicare and Medicaid programs.
HHS
Health & Human Services
Department of Health and Human Services (HHS) is to enhance the health and well-being of all Americans, by providing for effective health and human services and by fostering sound, sustained advances in the sciences underlying medicine, public health, and social services.
HCC
Hierarchical Condition Category
Risk adjustment model that groups similar diagnoses into one related category and is used by CMS to determine a risk score.
ICD-10
The International Classification of Diseases
Classification system for all physical and mental diseases. In Vatica this is broken down into three code lists, commercial, Medicare, and Medicaid that can be found in the VCC Resources tab.
WFG
VCC Workflow Guidelines
A required tool utilized to guide chart preparation.
RAF
Risk Adjustment Factor
Scores that each patient receives when risk adjusted.
MA
Medicare
- Mostly patients above 65
- Chronic, active conditions
- Usually have a higher risk adjustment score
MCD
Medicaid
A federal and state program that helps with healthcare costs for people with limited income and resources.
COM
Commercial
The open marketplace where individuals can choose to buy into government sponsored healthcare.
