When does carcinoid syndrome occur?
A
When mets to liver
May also occur in lung carcinoid as mediators not cleared by the liver

What is carcinoid syndrome?
A
Release of serotonin, kinins, histamines, PGs into the circulation

What are the SX?
A
Flushing (1) Diarrhoea Bronchospasm HypoTN R valvular stenosis LH affected in bronchial carcinoid Cushing's syndrome - as ACTH may also be released

Urinary 5-HIAA (breakdown product of serotonin)
Plasma chromogranin A y
Liver CT - mets

How is it MX?
A
Ocreotide (somatostatin analogue)
- inhibits release of hormones
Surgical resection
Diarhhoea - cyproheptadine
