Luminal cell layer - inner, responsible for milk production (in lobules)
Myoepithelial cell layer - outer, contractile cells

Inflammation of subareolar ducts
In smokers, because smoking causes relative Vit A deficiency which is required to maintain specialized epithelium, so you get squamos metaplasia in ducts causing blockage/inflammation
Presents as subareolar mass with nipple retraction (from fibrosis)
