Asthma In Children Flashcards
What is theMost common chronic respiratory disorder in childhood?
Asthma
TH1-type cytokines in asthma include
IL-2, IL-12, IL-18, and tumour
necrosis factor beta
TH2-type cytokines In asthma include..
(interleukins IL-4, IL-5, IL-9, IL-13) regulate
eosinophilia, mast cell growth, IgE and mucus production
What are the inhaler devices that you know?
Metered dose inhaler(MDI) +/- spacer
Dry powder inhaler (DPI)
Nebulizer
Treatment of acute asthma
• Oxygen • Beta 2 agonist nebulised/bolus • Steroids- oral/iv • IV infusion of: magnesium/aminophylline/salbutamol • ITU • Assisted Ventilation • Frequent REVIEW
Criteria for admission to hospital in asthma patient.
- Poor response to bronchodilator therapy
- Exhaustion
- Marked reduction in predicted (or usual best) PEFR or FEV 1 (<50%)
- Reduced oxygen saturation (<92% in air)
Management of Acute Asthma
High dose bronchodilator (salbutamol)
• MDI + spacer
• Nebulizer + Oxygen
• Ipratropium bromide : nebulizer
• Corticosteroids: oral e.g. prednisolone 3-7 days, parenteral or nebulized
• IV treatment for severe / Life threatening asthma
• IV Magnesium sulphate, aminophylline, or intravenous salbutamol
• Oxygen
• ICU Mechanical Ventilation
• Antibiotics only if infection
