SVT- definition
ANy tachycardia that is not ventricular in origin
Episodes characterised by sudden onset of a narrow complex tachycardia- typically an AVNRT
Other causes: AVRT and junctional tachycardias
Acute management of SVT
Vagal manoeuvres: valsalva and carotid sinus massage/.
IV adenosis 6mg-12mg-12mg
CONTRA IN ASTHMATICS- give verapamil
Electric Cardioversion
Prevention of SVT episodes
Beta-blocers
Radiofrequency ablation
Torsades de Pointes meaning
Torsades de pointes (‘twisting of the points’) is a form of polymorphic ventricular tachycardia associated with a long QT interval. It may deteriorate into ventricular fibrillation and hence lead to sudden death
Causes of Torsades de pointes
Causes of long QT interval congenital: Jervell-Lange-Nielsen syndrome, Romano-Ward syndrome antiarrhythmics: amiodarone, sotalol, class 1a antiarrhythmic drugs tricyclic antidepressants antipsychotics chloroquine terfenadine erythromycin electrolyte: hypocalcaemia, hypokalaemia, hypomagnesaemia myocarditis hypothermia subarachnoid haemorrhage
Management of Torsades
IV magnesium sulfate
