Heparin
Catalyzes anti-thombin causing an inactivation of thrombin and Xa etc
Parental administration (IV or IM)
Does NOT cross the placenta (Pregnancy is OKAY)
Half Life is Dose dependent
Does NOT effect synthesis of clotting factors
Does NOT lyse the existing clot
DOES prevent further clot formation
DOES prevent the further extension of the clot
Monitor via aPTT
Adverse Rxn
Bleeding or Thrombocytopenia
Used for DVT, Pulmonary embolism, coronary angioplasy or stent replacement. cardiopulmonary bypass
Low Molecular Weight Heparins

Does not effect thrombin
Renal Clearance
Used for DVT, hip replacement surgery, unstable angina and MI
Enoxaparin
Catalyzes anti-thrombin causing inactivation of Xa
Does not effect thrombin

Dalteparin
Catalyzes anti-thrombin causing inactivation of Xa

analog to Vitamin K that inhibits (competitive) VKORC1
Vit K then can not be recycled to assist in making functional clotting factors
Monitor with Prothrombin time and INR
Effects are delayed
Metabolized by CYP2C9
Adverse Rxn- Purple toe, skin necrosis
can overcome with Vit K
Used for- long term venous thromboembolic disease

Oral anticoagulant
Blocks active site of thrombin
inhibits fibrin bound and free thrombin
Rapid onset
Renal clearance
P-glycoprotien pumps into intestinal lumen
Used for- nonvalvular atrial fibrillation and knee or hip replacement
Rivaroxaban
Oral anticoagulant
Direct Xa inhibitor- binds to S1 and S4 pockets
decreases amplification of thrombin
inhibits Free Xa and Xa in the prothrombinase complex
P-glycoprotien pump
Renal clearance
Uses- nonvalvular atrial fibrillation and hip or knee replacement
Tissue Plasminogen Activating Factor (t-PA)
Plasminogen activator to Plasmin
Plasmin degrades clotting factors
Alteplase
Plasminogen activator to Plasmin
Clot buster
Plasmin degrades Fibrin
Activates bound plasminogen more readily than free
IV administration and short half life
Complications- Hemorrhage
Used for- Acute MI, Pulmonary embolism/DVT, Stroke (within 3 hours)
