A
Blood loss from menstruation
Pregnancy
Poor absorption

A
Chronic blood loss d/t gi malignancy, gi lesions, autoimmune gi disease
Poor intake
Malabsorption

Ix to diagnose IDA
A
CBC - hb low Serum ferritin (storage) - reduce Fe2+ (free) - reduce Transferrin (vehicle) - increase TIBC - increase

Perioperstive mx of anemia
A
Identify cause
Treat the cause
Treat anemia
Postpone non urgent elective surgery

Tx of anemia?
A
Oral iron supplements 4-8 weeks prior to surgery, until postop
200mg daily for therapy. Maintenance 40-60mg
IV iron indicated - urgent emergency surgery - unable to tolerate oral iron or unable to be absorbed - functional iron deficiency 20mg/kg
Blood transfusion
- symptomatic anemia (tachycardia, sob, pallor, bounding pulse)
- Hb < 7g/dL
EPO
- refuse blood transfusion
- blood not available
