Anaesthesia technique and complications Flashcards
What length needle is needed for ID block?
35mm
What length needle is needed for an infiltration?
25mm
How do you perform a long buccal?
Supra periosteal distal and buccal to last standing molar 2-4mm deep
What are the local complications arising form LA?
Pain on injection Post injection pain Failed anaesthesia Vascular problem Infection Interfere to with wound healing Self inflicted trauma Trismus
What can cause pain on injection?
Intra epithelial infection and sub periosteal injection
Too rapid
Direct contact with nerve trunk
What neurological effect may arise?
Prolonged anaesthisia due to nerve trunk damage Facial paralysis Auditory disturbance Visual disturbance Unilateral paralysis
Why may you get a Unilaterla paralysis?
Intravasculae injection
Which vascular problems may arise?
Intravascular injection
Intravenous infection
Heamatoma
Intrarterial
What will happen with an intra arteries injection?
Localalised blanching
What is the effect of LA in wound healing?
LA decreased cell motility
Adrenaline reduced Oxyhen tension
What are the systemic complications arising from LA?
Psychogenic effects Toxicity Drug interactions Allergy Methagammaglobunamia
What causes methagammaglobunaemia?
Prilocaine metabolites convert iron in Hb to ferric state this reduces oxygen carrying capacity
Can also occur with Articaine and benzocaine
What is the effect of lidocaine in suzamethinium?
Lidocaine prolongs it
What are the signs and symptoms of mild LA overdose ?
Talkative Apprehension Excitability Sweating Elevated BP Hr RR Disoritntion Light headed Nervous Twitching sensation LOC drowsiness
What are the signs of moderate to severe overdose?
Tonic clonic seizure followed by generalised CNS depression and depressed BP, HR, RR
