Outline the renin angiotensin system
A
Renin converts angiotensinogen to AT1. ACE converts AT1 to AT2

What are the effects of angiotensin 2?
A
Sympathetic activation, vasoconstriction, ADH and aldosterone release

A
Kidneys and lungs

A
Hypertension in under 55s
Heart failure with reduced ejection fraction

Afro-caribbean or elderly Impaired renal function or bilat RAS Pregnancy Previous angioedema Aortic stenosis

Side effects of ACE inhibitor
A
Dry cough- bradykinin
Angioedema
First dose hypotension
Hyperkalaemia

Other BP tablets Diuretics esp spironalactone NSAIDs - cause AKI Anticoagulants Gliptins

How do ACE inhibitors work?
A
Block angiotensin converting enzyme so angiotensin 2 not produced - sympathetic vasoconstriction/ADH/aldosterone blocked
